Student medication

For any medication to be administered to a student during school hours and/or during school-related events, the school must be provided with:

โ€ข a completed and signed Consent to Administer Medication form

โ€ข the medication with an attached current pharmacy label (that indicates that it is prescribed medication), in its original container, with intact packaging and/or

โ€ข written medical authorisation completed and signed by the prescribing health practitioner

Examples of medical authorisation include:

โ€ข a current pharmacy label with both the student's and doctor's names on it

โ€ข a signed letter from a doctor or other signed written instructions from the prescribing health practitioner
